Hours

The office is open Monday through Friday from 8:15am to 5:15pm. We are closed for lunch from 12:15-1:15 daily.

Cancellation Policy

We have a 24hr cancellation policy. All late cancellations and no show visits will be charged $15 for sick visits and $25 for well child checks.

We are happy to fill out school and immunization forms for your child. Please fax, mail, or drop off the form you need completed and allow 24 hours for completion.

The Seattle Public Schools Certificate of Immunization Status form can usually be filled out by you (if you have a record of your child's vaccines) and does NOT need a health care provider signature. A health care provider signature is only necessary if your child has had chicken pox or if he/she has had a blood test to confirm immunity to a vaccine preventable disease.
